By submitting this reservation, you are paying a non-refundable deposit to hold this reservation until the start date. You will receive a follow-up phone call after submitting this reservation, where you will be required to add your credit card to file. The remaining rental fee will be charged on the day of delivery at which time the deposit will be deducted from your first rental fee. Be sure to review all fees as listed below. Any rental add-on's and delivery fees will be charged at that time as well.

The deluxe rental is for The Dione infinite position lift chair. An infinite position chair allows you to adjust the head and foot rests independently, customizing the perfect position. Available in rich faux leather, this chair is extremely comfortable while offering the rugged durability of leather. Made from 100% polyester, the fabric is easy to care for ensuring functionality as well as elegance.

Deliveries are made on Monday, Tuesday, Wednesday and Thursday, during regular business hours. If you require delivery on a Friday, please call us and we will make every effort to accommodate.
Addresses more than 20 miles from our location require either a two month minimum rental, or an additional charge for pickup.
We recommend reserving at least 72 hours in advance but are often able to accommodate requests within 24 hours, especially in urgent situations.
If your reservation is submitted before 3:00pm on a business day, we will call you before 5:00pm to provide the timeslot for the delivery date. If reservation is submitted after 3:00pm or on the weekend, we will call you before 11:00am on the next business day.

Renter (Responsible Party) must be at least 18 years old, provide proof of identification, and pay stated security deposit and rental price in full prior to taking possession of equipment. Rental equipment must be returned by close of business on the scheduled return date, or renter will be charged for an additional rental period. Renter is responsible for the full cost of the rental period, regardless of early returns. Renter understands and agrees in the event that equipment is returned damaged or destroyed, Renter will be held financially accountable for any and all repair costs and/or total replacement of equipment. By signing this agreement, renter pre-authorizes the credit card on file to be charged for any repair or replacement costs.
1. The medical equipment is the property of MI MED, Inc. and is in good condition. Renter shall return equipment in the same condition as when received. The equipment may be repossessed if it is used in violation of the terms of this agreement.
2. Renter shall defend, indemnify and hold harmless MI MED, Inc., all agents and employees from and against any and all liability claims, injuries and causes of action, related to any claim by persons that may arise from the operation of the equipment, provided that such incident was not caused by the fault, negligence or misconduct of MI MED, Inc. or employees.
Please review your order carefully before submitting. When there is a cancellation or refund, we incur a 3% fee from our website platform. We will deduct that fee from your refund unless it is due to our error. This applies to online orders and rental reservations, it does not apply to in-store purchases.
